REBATE PROGRAM

To qualify for a $25.00 public rebate from Brevard County after your dog or cat has been sterilized, you must meet the following requirements:

    You must be a resident of Brevard County and provide:

                Proof of Residency - photocopy of a Florida State Drivers License or ID
                Proof of Rabies Vaccination - photocopy of vaccination certificate
                Proof of Brevard County Animal License Tag
                Proof of Sterilization

                *Rabies vaccination and Brevard County Animal License Tag must be current. Only licenses issued by
                  Brevard County will be honored.

                *Proof of sterilization must be in the form of an invoice that includes the name, address and phone
                  number of the veterinarian/clinic that performed the procedure, the name of the procedure, cost of
                  the procedure, owners name and animal's name.

          The rebate program applies to "owned" companion animals only.

        You must file a REBATE APPLICATION within 30 days of sterilization to be eligible for the $25.00
                rebate.  Procedures less than $25.00 will be paid at 100% reimbursement for surgery.

        Residents adopting animals from the Brevard County Animal Care Centers, the SPCA or the Central
               Brevard Humane Society are not eligible for the Rebate Program as the price of sterilization is included
               in the adoption cost.

        No more than four (4) animals per year/per household can qualify for the rebate program.

        Rebates are limited to available resources.

        PLEASE SEND COPIES OF DOCUMENTS ONLY. DOCUMENTS WILL NOT BE RETURNED.

       Applicants will be notified if they do not qualify for the rebate.
